At Reach Me, Inc. we start by having a conversation. Our dedicated staff are eager to learn about the impacted adult and how we can best serve their needs for friendship, community and inclusion. It is our belief that caregivers, family members and the person impacted by a disability are the subject matter experts when it comes to living their best lives.

Below is a list of topics you may want to discuss with your loved one and their care team.

What does a typical week look like right now?
What appointments, programs, or providers are already involved?
What has been hardest lately?
Has anything changed with sleep, behavior, mood, or routine?
What does your loved one enjoy when they feel safe and supported?
What would “better” look like for your family, client, or care team?

WHAT IF WE don't have a medicaid waiver?

Whether your loved one has a Medicaid Waiver or they are a Non-Waiver clients, everyone is accepted at Reach Me, Inc. Simply let us know during our initial conversation and we will help you decide which pathway is right for your family's unique situation.

What should my loved one expect?

Our team continuously works with the caregiver, adult, and involved professionals to identify what will help the person feel safe, comfortable, and included. We have a robust schedule of planned events and provide transportation to each venue. Your loved one will bring a packed lunch to many of our excursions, but occasionally, we will cover their dining costs. Please review the posted schedule to see when a "packed lunch" is needed.

Do you accept referrals from other agencies?

We support professionals working with impacted adults in a variety of ways. A referral from your agency is always appreciated. If you are unsure if Reach Me, Inc. is the right fit for your client's needs, please do not hesitate to reach out.
